| The Alumni Information Entry allows you to give the CHMS department your current contact information. You'll have the opportunity to post a bit of your information on the CHMS Alumni site for other Alumni to see, or send it only to the CHMS Department for their records. |
| If you haven't signed up yet, select New Form. If you're here to update your information, select Updated Form, then type in your last name (capital letters count!) and your password from the last time you were here for verification. If, perchance, you can't remember that password, e-mail chmsweb@ucdavis.edu and your password will be emailed to you as soon as possible. |
